Installation Guide

1. BEGIN INSTALLATION: Prerequisites & Wabbajack
Log into Nexus in Wabbajack

Open Wabbajack, click the ⚙️ cogwheel, and log in under the Nexus panel.

To prevent interference, add a folder exclusion for your entire install directory:

  1. Open Windows Security → Virus & threat protection

📥 Downloading Fallout Anomaly

  • Locate Fallout Anomaly in the Wabbajack Browser and download the file.
  • Set both Install Location and Download Location on the same drive for best performance.

Could Not Download Mod

  • Fix: Manual Download - Download the mod manually and place it in Wabbajack's download folder.

Mod Is Not a Whitelisted Download

  • Wait: For a new release – The modlist may have been updated, making an old mod link invalid.
  • Contact: Notify us on Discord if the issue persists.

Missing Game Files

  • Update: Ensure your game is updated to the next-gen version.

Wabbajack Could Not Find My Game Folder

  • Verify: Pirated versions are not supported.
  • Ownership: Ensure you own Fallout 4 on Steam.

ENB Download Issues

  • Manual Download: If your ENB download fails, download it manually and move it to the Anomaly Downloads folder.
  • Note: This folder is created by Wabbajack during installation.
  • Warning: DO NOT extract the file—just place it in the folder.
  • Action: Restart Wabbajack and run the installer again.

[DISPLAY] Resolution & Visual Issues
→ Users running 1080p (1920x1080) resolutions shouldn't run into any issues
→ Issues more commonly occur with 1440p or higher resolutions
Super and ultra-wide monitors require the provided patches to be active in Mod Organizer 2
Refracted/Boxed Display (Screen in Upper Left Corner)
This issue has multiple causes related to .ini settings interacting with your Windows resolution.
Check each step in order and launch the game after each to see if the issue is resolved:
Step 1: Check Third-Party Software
→ Ensure you are NOT running any 3rd party software to adjust your resolution
→ These applications are always considered incompatible with the modlist
Step 2: Verify Monitor Settings
→ Check that your monitor is running at its native resolution
→ This is for troubleshooting purposes - you can change it back later
Step 3: Set Correct Resolution in BethINI
IMPORTANT: Close MO2 before running BethINI
→ Run BethINI as an administrator
→ Set the correct resolution for your monitor
→ Save and close BethINI
Step 4: Verify Display Mode Settings
→ Open Mod Organizer 2
→ Navigate to your Falloutprefs.ini file in MO2
→ Verify that you have either bFullscreen=1 or bBorderless=1 set
→ Only one should be set to 1, not both
Step 5: DPI Scaling Override (Last Resort)
If all above steps fail, try this DPI scaling fix:
1. Navigate to your Stock Folder inside your Installation Directory
2. Right-click on Fallout4.exe and select Properties
3. Navigate to the Compatibility tab
4. Click on "Change High DPI settings"
5. Check the box for "High DPI Scaling Override"
6. Ensure the drop-down menu is set to Application
7. Click OK and Apply

After Installation

Congratulations, you've installed Fallout Anomaly! Before you jump into the game, there are a few essential steps to complete to ensure everything runs smoothly.

Launching Fallout Anomaly

1. Launching Fallout Anomaly

To start the game with all mods properly loaded, click the "Launch Fallout Anomaly" button in the top-right corner of the MO2 interface.

Warning: Always launch the game through Mod Organizer 2. Launching directly from Steam will not load any mods.

2. Crash Handling

If your game crashes, a crash log will be generated in your Overwrite folder. You can use this log to diagnose the issue. Please report crashes via our Discord or bug report form.

3. Changing Resolution

To change your resolution, use the BethINI tool. Close MO2, run `BethINI.exe` as an administrator from the `Tools` folder, set your resolution in the Basic tab, and save.

4. Apply the MCM Preset!

Once you are in-game, you will be prompted to apply the MCM (Mod Configuration Menu) preset. This is a critical step to ensure all mods are configured correctly.

5. Disabling Black Bars

If you don't like the cinematic black bars (letterboxing), you can disable them via the ENB menu.

  1. Press the `~` key to open the console.
  2. Press `END` to open the ENB menu.
  3. In the Post Processing section, uncheck the "Letterbox" option.
  4. Click "Save Configuration" in the top-left.
  5. Press `END` and `~` again to close the menus.

Last Resort: DPI Scaling Fix

If the above steps fail, try this before making a support post:

  1. Go to your Stock Folder inside your Installation Directory
  2. Right-click on Fallout4.exe and select Properties
  3. Navigate to the Compatibility tab

Known Issues

Main Menu Delay

  • When launching the game you may experience a delay as scripts load
  • This delay varies based on your system's power
  • More powerful systems may not encounter this issue

Stuck in Menu

  • If you find yourself unable to progress from the first loading or main menu
  • Watch this video for help with the issue
  • If the video doesn't resolve it, we've provided new save files for you to load and customize your character as needed

Post-Character Creation Freeze

  • After creating your character, your game will save and may appear to freeze temporarily
  • This is due to multiple scripts loading simultaneously
  • Be patient and wait for a few minutes - the game will resume normally

Falling Through Vault Issue

  • If you're falling through the vault floor, this is likely due to the "Uneducated Shooter" mod
  • Solution: Only enable the "Uneducated Shooter" mod after leaving the vault

Known Crash Related Issues

  • Programs like Medal (recording software) or anything that adds an overlay can conflict with ENBs or other DLL hooked mods
  • If you are crashing right at the launch of game or at the main menu, this can be one of the main causes

  • Ensure your Steam controller settings are correctly set up as shown in the images below
  • Steam Overlay must be enabled for the controller to function properly
  • Steam Input: Paste the following link into your browser and open Steam when prompted:

Step 2: Enable the Controller Support Mod

  • In MO2, activate the optional mod:

Gameplay Guide

1. CORE SURVIVAL: Needs, Healing, & Radiation
🍽️ Survival Mode Features
Needs System: You must regularly eat, drink, and sleep to avoid penalties. Hunger, thirst, and fatigue will reduce your stats and eventually cause damage or even death
Fast Travel: Traditional fast travel is disabled. You can only fast travel by using the APC Travel System or navigating the expanded subway system found across the Commonwealth
Saving the Game: Manual saving and quicksaving are disabled. You can only save by smoking, using Survival Pack item found in the Aid menu or by sleeping.
Combat Difficulty: Both you and enemies deal more damage. Healing is slowed, and combat is more lethal across the board
Ammo Has Weight: Every bullet, shell, or fusion cell you carry adds weight to your inventory, forcing smarter inventory management
Illness & Injury: You can get sick, suffer broken limbs, or require items like antibiotics and splints for proper healing. Doctors may be needed to cure serious conditions
Carry Weight Reduced: Your base carry weight is significantly lower, making every item choice matter. Backpacks and perks help extend capacity
Backpacks: Can be found or crafted using perks from the Junk Jockey skill to increase carry capacity

Healing Guide - MAIM Medical System

MAIM is a realistic combat and medical overhaul that introduces lethal headshots, injuries, bleeding, pain, and more. It is highly customizable through the MCM, allowing most features to be tweaked or disabled. The system remains performance-friendly with minimal scripts.

This is a major gameplay overhaul. We recommend reading the full mod page on Nexus Mods. A solid MCM setup is pre-configured for you out of the box.

SP-1 Stimpak: Gradual HP + AP regen, cures NPC injuries, strong poison resistance (180s)
SP-2 Stimpak: 3%/s HP & limb healing (40s). Most powerful heal, but requires weapon holstered
Med-X: Treats pain, shock, and contusion. Heals head 1%/s (180s). Addictive
Ibuprofen: Heals head & non-crippled limbs 0.5%/s (180s). Mildly addictive
Bandage: Stops light bleeds, heals injured limbs (1%/s for 60s)
QuikClot Injector: Stops heavy bleeds and prevents further bleeding until limb is healed
IFAK: Stops heavy bleed, removes crippled status, heals 1%/s (80s)
SAM Splint: Sets crippled limbs to 1 HP, downgrades heavy bleed

Pro Tip: Healing items do not stack with themselves, but you can combine different effects (e.g., SP-1 + SP-2). Pre-med with TXA and ibuprofen before a firefight for best results.

Radiation System - Advanced Mechanics
⚛️ Radiation Mechanics
Dual-layered System: RAD (Tissue Damage) from environmental exposure + IRA (Ingested Radioactive Particles) from food/water/weather/hazards. These are two separate but interconnected systems that can feed into each other.

The Radiation Loop

Understanding the vicious cycle of radiation damage and IRA acquisition is crucial for survival. This diagram illustrates how radiation exposure can lead to a compounding negative feedback loop that gets worse without intervention.

1. Initial Exposure

Environmental radiation from zones, food, water, or hazards begins damaging your tissues.

2. RAD Damage Accumulates

Without treatment, radiation damage builds up, reducing your maximum HP and overall health.

3. IRA Increases

Internal Radioactive Absorption rises, making you more susceptible to future radiation damage.

4. Vulnerability Spiral

Higher IRA means more damage from the same exposure, creating a deadly feedback loop.

  • Proper treatment with radiation medicines can interrupt the progression before it becomes fatal
  • Use protective gear to prevent initial exposure
Fatal Progression: Untreated radiation will eventually kill the player—even leaving radioactive areas doesn't guarantee safety once the loop has started. The damage continues to compound until treated.
Geiger Alerts: Your Geiger counter will indicate ambient particles, radiation poisoning, and dangerous IRA levels. Pay attention to these warnings—they're your early warning system.
Weather Hazards: Radiation storms require functional protective gear (gas masks, hazmat suits, power helmets). Without proper protection, you'll quickly accumulate both RAD and IRA damage.

The Wastes are unforgiving. Radiation is no longer a simple number—it's a dual-layered system where RADs and IRAs feed into each other, creating a deadly loop that can take hold in seconds or over days. Untreated radiation will eventually kill you, even if you leave radioactive areas. Always carry enough protective gear and medication. Stock up on bandages, painkillers, radiation medicines (especially Cleansing Tonic for radiation poisoning), and limb restoration tools. Remember: consuming alcohol can help break the radiation loop by lowering IRA levels. Being prepared isn't optional—it's survival.
